Conveniently located in Southridge of Corridor G, our office is on the second floor of the Pied Piper building near Office Max (see map below).
Parking is free. Enter through the door under the green awning, take the elevator to Suite 202, and have a cup of coffee.
You can print your registration forms here and bring them with you to your first visit, or simply arrive at our office at least 15 minutes early to complete your registration.

Monday, Wednesday, Friday: 8:00 am - 5:00 pm
Tuesday and Thursday: 8:00 am - 6:30 pm
We are understanding of your schedule, and we like to be as flexible as possible. If you have concerns about your appointments, CPT will do our best to accommodate your schedule.
Your First Visit
Paperwork can be a pain . . . although a necessary one. To save yourself time at your first appointment, you can access the necessary forms here to complete at home prior to your arrival at our office.
Time
Please plan for your initial appointment to take at least one hour. Our team wants to give you the attention you deserve. This appointment will give the therapists the information to assess and evaluate the treatment that is best for your needs.
